Özet:1. İstanbul’da temel sağlık hizmetlerindeki sağlık bilgi sisteminin belli bir kapsamı ve planı bulunmamaktadır. Sağlık bilgi sistemini oluşturan argümanlar, kurumlar arası koordinasyon ve işbirliği olmaksızın ve bir altyapıya dayanmaksızın toplanmaktadır. Bu bilgilerin toplanmasında da benzer şekilde belli bir algoritma uygulanmamakta ve bilgilerin bir kısmı yinelenerek toplanırken, bir kısmı da hiç bir değerlendirmeye tabi tutulmaksızın kaydedilmektedir. Bazı bilgiler ise ne kaydedilmekte ne de değerlendirilmektedir. Bu çalışmanın amacı, İstanbul’da, temel sağlık hizmetleri açısından sağlık bilgi sisteminin durumunu ortaya koymak ve bu sistem içindeki sıkıntıları belirleyerek, kapsamlı sağlık bilgi sistemi bulunan bazı ülkelerle kıyaslayarak, İstanbul’da temel sağlık hizmetlerinde ideal bir sağlık bilgi sistemi oluşturmak için öneri getirmektir. Buna yönelik olarak, İstanbul’da sağlık ocakları, ana çocuk sağlığı ve aile planlaması merkezleri, verem savaş dispanserleri, kamu hastanesi poliklinikleri, belediyeler, iş yeri hekimleri, hıfzıssıhha laboratuarları, özel ve vakıf poliklinikleri ile özel muayenehanelerde kayıt altına alınan veriler incelenmiş, bu kayıtların hangi periyotta, hangi makamlara iletildiği belirlenmiş, bu iletilen verilerin nerelerde ve nasıl incelendiği tespit edilmiş, verilerden geri bildirim yapılanların tespitleri yapılmıştır. Araştırmanın sonunda, hiç bir şekilde kayıt altına alınmayan durumlar belirlenmiş, kayıt altına alınıp, her hangi bir aşamada inceleme ve değerlendirmeye tabi tutulmayan veriler tespit edilmiş, temel sağlık hizmetlerinde sağlık bilgi sistemindeki aksaklıklar ortaya çıkarılmış ve sağlık bilgi sistemi düzenli olan ülkelerle kıyaslamalar yapılmıştır. Bu araştırma sonucunda, özellikle temel sağlık hizmeti sunan kurumlara ait verilerin değerlendirilmesinde koordinasyonun yetersiz olduğu, özellikle Bakanlığa bağlı olmayan sağlık kurumlarında gerçekleştirilen kayıtlardan Bakanlığın haberdar olmadığı, pek çok kurum tarafından, hasta kayıtlarının tekrarlandığı, sürveyansa ait kayıt sisteminin yetersiz olduğu, veri kaydının ilk aşamasından, verilerin aktarıldığı son aşamasına kadar giden yolda geri bildirimlerin düzenli yapılmadığı ve büyük ölçüde bu nedenle, bazı formların bazı bilgilerinin özensiz girildiği tespit edilmiştir. SUMMARY Health Information System in Primary Health Care There is not a certain concept and plan for the health information system in basic health services in Istanbul. The arguments which forms the health information system have been collected without any coordination and cooperation of the related institutions and without any basic structure. In the collection of this information, there is not any algoritm so, while there is duplication for some data, some of the others have been recorded without any analysis. Some of the information are neither recorded nor analized. The aim of this study is to find out the situation of the health information system in Istanbul and determining the problems of this system by comparing it with the other countries who have a rich health information system in order to present proposals for an ideal health information system for the basic health services in Istanbul. For this aim, the recordings in the primary health offices, in mother and child health and reproductive health services centers, common hospital clinics, local government health offices, company health offices, special and common health clinics have been analized; the periods and the monitorizing of the recordings have been inspected; the analizing methods and places have been determined and the recordings which are used for feedbacks have been described. At the end of the study, the situations which have not been recorded are determined, the recordings which have not been examined or evaluated in any period are described, the problems in the health information system in basic health services are found out and the countries which have regular health information systems are compared with ours. It is found out that; the coordination in evaluating the recordings of institutions who serve for primary health care is not enough, Health Ministry is not informed in any way about the recordings of the health services which are not in the concept of the Ministry, the recordings of the patients are dublicated by different health care services, the recording system of the surveyance is very poor, the feedbacks are not applied well from the beginning of the recordings till the end of the last point and mostly because of this, it is observed that some recordings of some forms are recorded without attention.
