Epizyotomi sonrası perineye buz uygulamasının ağrıyı azaltmaya ve yara iyileşmesine etkisi

Özet:

EPİZYOTOMİ SONRASI PERİNEYE BUZ UYGULAMASININ AĞRIYI AZALTMAYA VE YARA İYİLEŞMESİNE ETKİSİ, ELİF VELİOĞLU Araştırma, epizyotomi ile vajinal doğum yapan kadınlara, doğum sonu dönemde perineye buz uygulamasının ağrının azaltılmasına ve yara iyileşmesine etkisinin incelenmesi amacıyla deneysel olarak planlanmıştır. Örneklem grubuna Bursa Çekirge Devlet Hastanesi Kadın Doğum Kliniğine 1 Ağustos – 1 Ekim 2013 tarihleri arasında başvuran mediolateral epizyotomili vajinal doğum yapan, perine yırtığı olmayan, yenidoğan ağırlığı 2500-4500 gram arasında olan 73 anne deney, 75 anne kontrol grubuna alındı. İki gruptaki annelere doğumdan hemen sonra ilk olarak Veri Toplama Formu, REEDA Skalası ve GKÖ(Görsel Kıyaslama Ölçeği) uygulandı. REEDA ve GKÖ annelere 1. saatte, 3. saatte, 24. saatte ve 7. günde tekrar uygulanmıştır. Deney grubundaki annelere ilk 3 saat, 10 dakika ara ile 20 dakika boyunca perine bölgesine buz uygulaması yapıldı. Kontrol grubundakilere ise hiçbir uygulama yapılmamıştır. Veriler Mann-Whitney U testi, Wilcoxon testi, Spearman korelasyon analizi, Kruskal Wallis testi ve Fisher’in kesin ki-kare ile analiz edilmiştir. Araştırmanın sonucunda kontrol grubundaki annelerin GKÖ, REEDA ve Ödem 1. saat ve başlangıç(0.dk.) değerleri arasında anlamlı fark yokken(p>0.05), deney grubundaki annelerin GKÖ, REEDA ve Ödem 1. saat ve başlangıç değerleri arasında anlamlı fark vardır(p<0.001). Deney ve kontrol grubu arasında ise GKÖ, REEDA ve Ödem fark skorları bakımından ileri düzeyde anlamlı fark vardır(p<0.001). Ayrıca travay süresi ve yenidoğan ağırlığı ile ağrı, ödem ve yara iyileşmesi arasında pozitif yönde korelasyon saptanmıştır. Sonuç olarak epizyotomi sonrası perine bölgesine buz uygulamasının ağrıyı, ödemi azalttığı ve yara iyileşmesini hızlandırdığı saptanmıştır. Özellikle ilk bir saatte yapılan buz uygulaması ağrı, ödem ve yara iyileşmesi üzerinde en yüksek etkiye sahiptir. Anahtar kelimeler: epizyotomi , buz uygulaması, ağrı, yara iyileşmesi, REEDA skalası SUMMARY The Effect of Applying Ice on the Perineum following an Episiotomy on Pain Reduction and Wound Healing, Elif VELİOĞLU The study was planned as experimental research to examine the effect on pain reduction and wound healing of applying ice on the perineum of women undergoing an episiotomy during vaginal delivery. The sample comprised women who had vaginally delivered infants of a birth weight of 2500-4500 grams with a mediolateral episiotomy between the period August 1 – October 1, 2013 at the Bursa Çekirge State Hospital Obstetrics and Gynecology Clinic; of the mothers, 73 were recruited into the study group and 75 into a control group. A Data Collection Form, the REEDA and the Visual Analogue Scales (VAS) were administered to the mothers immediately after delivery. The REEDA and VAS were re-administered at the 1st, 3rd, 24th hour and on the 7th day. Ice was applied to the mothers in the study group for 20 minutes in the first 3 hours at 10-minute intervals. No ice was applied to the mothers in the control group. The data were analyzed using the Mann-Whitney U test, the Wilcoxon test, Spearman's correlation analysis, the Kruskal-Wallis test and Fisher's exact test. At the end of the research, it was found that the mothers in the control group did not display any significant difference between their VAS, REEDA and Edema baseline (0 min.) and 1st hour scores (p>0.05), whereas the mothers in the study group exhibited significant differences between their VAS, REEDA and Edema baseline and 1st hour scores (p<0.001). There was a significant difference between the study and control groups in terms of the difference in their VAS, REEDA and Edema scores (p<0.001). In addition, a positive correlation was found between the length of labor and the newborn's birth weight and pain, edema and wound healing. The conclusion was drawn that ice applied to the perineum area following an episiotomy reduces pain and edema and hastens wound healing. The application of the ice in the first hour in particular was seen to produce the greatest effect on pain, edema and wound healing.
