Mitokondriyal MOTS-C geni nükleotit varyasyonlarının nonalkolik yağlı karaciğer hastalığı etyopatogenezi ile ilişkisi

Özet:

Amaç: 2015 yılında mtDNA’ya ait yeni bir gen keşfedilmiştir. Bu genin fonksiyonel ürünü olan MOTS-c peptidinin deneysel hayvan modellerinde insülin direncini ve obeziteyi azalttığı bildirilmiştir. MOTS-c genindeki herhangi bir varyasyonun nonalkoliksteatohepatit (NASH) etyopatogenezindeki temel mekanizma olan insülin direncinden sorumlu olabileceği düşünülebilir. Bu çalışmamızın amacı MOTS-c genindeki olası tüm varyasyonların NASH ile muhtemel ilişkisini araştırmaktır. Gereç ve Yöntem: Bu çalışmaya 100 NASH tanılı hasta ve 100 sağlıklı kontrol alındı ve bireylere ait periferik kanlardan DNA izolasyonu yapıldı. MOTS-c genin (GenBank: KP715230.1) amplifikasyonu için Primer3 programı kullanılarak primerler tasarlandı. ABI3500 sekans cihazında sanger yöntemine göre tüm MOTS-c geni dizilendi. Varyasyon (mutasyon/polimorfizm) analizi için NCBI/BLAST programı kullanıldı. Bulgular: Sekans analiz sonuçlarına göre yalnızca bir hastanın MOTS-c geninde mutasyon tespit edilmiştir. Kontrol grubunda ise herhangi bir varyasyona rastlanmamıştır. Sonuçlar: mtDNA oldukça varyant olmasına rağmen MOTS-c geninin büyük ölçüde korunmuş olduğu saptanmıştır. Literatürde ilk kez NASH hastalarında stop kodonunu, glisin aminoasidine dönüştüren nonsinonim MOTS-c geni mutasyonu tespit edilip; “GenBank” ve “MİTOMAP”e yeni bir mutasyon olarak rapor edilmiştir. Bu mutasyon normalinden daha uzun bir non-fonksiyonel peptid oluşumuna neden olabilir. Bu durumda iskelet kaslarında, MOTS-c’nin non-fonksiyonel türevi AMPK yolağını aktive edemez ve kan glikoz seviyeleri artıp insulin direnci gelişebilir. Saptadığımız mutasyonun daha geniş serilerde araştırılmasında fayda olduğunu düşünmekteyiz. Bu mutasyonun MOTS-c peptid üretimi üzerine etkileri in vitroda deneysel olarak incelenebilir. ABSTRACT Objective: In 2015, a new gene called as MOTS-c was found in 12S rRNA region of mtDNA. It has been reported MOTS-c peptide reduces insulin resistance and obesity in experimental animal models. So we thought that a variation in the MOTS-c gene may be responsible for insulin resistance in nonalcoholic steatohepatitis (NASH). In this study, we aimed to find all possible MOTS-c gene variations in NASH and associate them with NASH. Methods: DNA was isolated from the peripheral blood of 100 NASH patients and 100 healthy controls. Suitable primers were designed by using the Primer3 program for amplification of the MOTS-c gene (GenBank: KP715230.1). MOTS-c gene sequencing was performed based on sanger method by using Applied Biosystems 3500 Genetic Analyzer. NCBI / BLAST program was used for analysis of variation. Results: According to the sequence analysis results, a variation was detected in the MOTS-c gene of one patient. There is no variation was found in the control group. Conclusion: We found that the MOTS-c gene is quite conserved, although mtDNA is highly variant. We detected a mutation which converts stop codon to glycine aminoacid in the MOTS-c gene of one NASH patient. This mutation may result a non-functional peptide formation longer than MOTS-c. In this case, nonfunctional peptide can not activate AMPK pathway so blood glucose levels may increase and insulin resistance may develop in skeletal muscles. This mutation should be researched in wider NASH groups and the effect of this mutation on MOTS-c gene production should experiment in vitro.
