Evaluation of treatment outcome and long-term stability of class II correction by forsus fatigue resistant device

Özet:

Amaç: Bu tez çalışmasının amacı Forsus “yorgunluk dirençli aparey” (FRD) ile Sınıf II maloklüzyon düzeltiminin uzun dönem etkilerinin retrospektif bir klinik çalışma ile değerlendirilmesidir. Gereç ve yöntem: Sınıf II maloklüzyona sahip (Grup A: bölüm 1, n=9; Grup B: n=11) ve Forsus apareyi ile sabit fonksiyonel ortodontik tedavi görmüş 20 hastanın (ortalama yaş: 17.3  2.5 yıl; 7 erkek 13 kadın) kayıtları Marmara Üniversitesi Diş Hekimliği Fakültesi Ortodonti A.D arşivinden elde edilmiştir. Uzun dönem iskeletsel ve dişsel değişimleri değerlendirmek amacıyla, tedavi sonunda (T1) ve takip dönemi (T2: ort.19  3 ay) sonrasında alınmış olan lateral sefalometrik röntgen kayıtları NemoCeph bilgisayar programında analiz edilmiş ve çakıştırılmıştır. İstatistiksel değerlendirme için bağımlı örneklem t testi ve Grup A ve Grup B için bağımsız örneklem t testi kullanılmıştır. Bulgular: Takip dönemi sonrasında çalışma grubunda sagital ve vertikal yönde istatistiksel olarak anlamlı bir iskeletsel değişim olmamıştır. Overjet miktarında anlamlı bir değişim bulunmuştur (+ 0,29 mm). Gruplar arası karşılaştırmada T1 ve T2’de IMPA değerinde anlamlı fark bulunurken ( sırasıyla 5,38 - 5,34) stabilite açısından fark bulunmamıştır. Sonuç: Forsus FRD uygulanması etkili olduğu ve sabit ortodontik tedavinin tamamlanmasını takiben ortalama 19 ay sonra tedavi sonuçlarının kalıcı olduğu tesbit edilmiştir. -------------------- AIM: The purpose of this thesis study was to investigate the long-term stability and the possible relapse of the Class II Correction following the use of Forsus (FRD) in a retrospective clinical study. Materials and Methods: The records of 20 patients (mean age 17.3  2.5 years, 7males 13females) with Class II malocclusion ( GroupA: division1, n:=9, GroupB: division2 n=11), who had undergone fixed functional treatment protocol, were collected from the archive of the Department of Orthodontics, Faculty of Dentistry, Marmara University. Lateral cephalometric radiographs that were taken at the end of comprehensive treatment(T1) and at a postretention period (T2:19months3) were traced and superimposed on NemoCeph program in order to assess skeletal and dental changes on the long term. Statistical comparisons were carried out with paired Samples t test for the whole sample and independent samples t test for GroupA & GroupB. Results: After T2, the sample exhibited no significant sagittal or vertical skeletal changes, while significant change was recorded only in overjet (+0,29 mm). The two groups comparison shows that a significant difference was registered for IMPA (-5,38&-5,34) at T1 and T2 respectively. However, no significant difference in treatment stability was recorded. Conclusion: The Forsus (FRD) treatment protocol was stable when evaluated 19 months after the end of comprehensive treatment
