Özet:

Kandida Vajiniti Gelişmiş Olan Fizyolojik ve Cerrahi Menopozlu Kadınlardan İzole Edilen Kökenlerde Fenotipik Dönüşüm ve Flukonazol Direncinin Araştırılması Nil TÜRKÖLMEZ Cerrahi menopozlu hastalarda fizyolojik menopozlu gruba oranla tekrarlayan ve tedaviye dirençli kandida vulvovajinitinin (VVK) daha yaygın olduğu bilinmektedir. Fakat bu farklılığı açıklayacak, mantara özgü virülans faktörleri hakkında bilgi bulunmamaktadır. Bununla beraber VVK enfeksiyonlarında etken kandidaların en önemli virülans faktörlerinden birisinin fenotipik dönüşüm olduğu bildirilmiştir. Bu veriden yola çıkarak çalışmamız, fizyolojik (n=30) ve cerrahi (n=30) menopoz döneminde VVK gelişen hastalardan izole edilen kökenlerin “fenotipik dönüşüm” özelliklerini; bunun flukonazole karşı olası direnç ile bağlantısını araştırmak üzere planlanmıştır. Fizyolojik menopoz grubu kökenlerinin sadece üçte birinin, cerrahi grup kökenlerinin ise yarısının fenotipik dönüşüm geçirdiği saptanmıştır. Bu bulgu cerrahi gruptaki olası rekürrens epizodlarını açıklayabilir. Fizyolojik menopozlu grupta C.albicans kökenlerinin %35,7’ si fenotipik dönüşüm geçirirken, albicans dışı kökenlerde bu oran %50’ dir. Cerrahi grupta ise albicans dışı kökenlerin fenotipik dönüşüm geçirme oranının, C.albicans kökenlerine göre üç kat fazla olduğu belirlenmiş ve istatistiksel açıdan da anlamlı bulunmuştur (P<0.05). Çalışma kökenlerinde ortaya çıkan fenotipik dönüşüm tipleri arasında en sık rastlanan CD fenotipi olmuştur. Fizyolojik menopoz grubunda sadece 6 kökenin (%20), cerrahi menopoz grubunda ise 13 kökenin (%43,3) CD koloniler oluşturduğu belirlenmiş ve gruplar arası bu farklılığın istatistiksel açıdan anlamlılık sınırında olduğu saptanmıştır (P=0.05). Ek olarak yüksek düzeyde CD fenotipi dönüşümü gösteren kökenlerin flukonazol MİK değerlerinde de yükselme eğilimi olduğu gözlenmiştir. (Trend Chi-kare, P<0.001). Sonuç olarak, fizyolojik ve cerrahi menopoz sonrası VVK’ larda, etkenin fenotipik dönüşüm yeteneğinin; enfeksiyonun patogenezinde, ve tedaviye yanıtsızlıkta önemli olabileceğini düşünmekteyiz. Anahtar Sözcükler: Fenotipik dönüşüm, Flukonazol, Kandida Vulvovajiniti, Menopoz SUMMARY Investigation of Phenotypic Switching and Fluconazole Resistance in the Strains Isolated From Candidal Vaginitis in Women with Physiological and Surgical Menopause Writer: Nil TÜRKÖLMEZ Hysterectomized post-menopausal patients are known to develope more frequent recurrent Candida Vulvo Vaginitis (VVC) infections that are more resistant to treatment when compared with non-hysterectomized group. Nevertheless, there is no knowledge about the virulence factors of the fungus that would explain this difference. However, in the studies relating infections in reproductive age, one of the most important virulence factors of Candida has been reported to be “phenotypic switching” . Based on this knowledge, this study was planned to investigate the phenotypic switching property and its probable relation with fluconazole resistance of the isolates from the hysterectomized (n=30) and physiological (non-hysterectomized) (n=30) patients with VVC. In the physiological group 1/3 and in the hysterectomized group 1/2 of the isolates were detected to exhibit phenotypic switching. This finding can explain the possible recurrence episodes in hysterectomized group. In physiological group the rate of phenotypic switching was 35,7% in C.albicans strains while 50% in non-albicans strains. In the hysterectomized group phenotypic switching rates of the non-albicans strains was three times greater than that of C.albicans and this difference was found to be statistically significant (P<0.05). The most frequent switching phenotype was “Dwarf Smooth” (DS) type of colonies among other phenotypes. In the physiological group only six isolates (20%) and in the hysterectomized group 13 isolates (43,3%) exhibited DS type of colonies and this difference was found to be on borderline statistical significance (P=0.05) In addition we observed that in the strains which produced high percentage of DS colonies there was also a trend of increase in fluconazole MIC values (Trend Chi-square Test, P<0.001) In conclusion we may suggest that phenotypic switching may be an inportant phenomenon in the pathogenesis of the infection and in unresponsiveness to the treatment in physiological and hysterectomized post-menopausal patient groups with VVC.
