In tooth-supported overdenture (TSO), the preparation of abutment teeth varies according to the intended use of abutment teeth. Preparation is made all around the tooth, and the prepared surface should be convex whenever used. TSO treatment concept is not a new concept, and clinicians have successfully employed existing tooth structures or retained roots to assist with complete denture treatment for more than a century. Two or more coronally modified or restored dental abutments are often treated endodontically and used as an abutment for overdenture to preserve alveolar ridge morphology and prevent bone loss. The preservation of supporting teeth as abutments provides efficient prosthetic treatment. This type of denture is the most suitable alternative on an economic basis among the treatment alternatives. The aim is to distribute stress concentration between retained abutments and denture-supporting soft tissues. Root retention offers various benefits: alveolar bone maintenance, better prosthesis support, proprioceptive feedback, aesthetics, and psychological benefits. In case of any complication, modification is easy on both the supporting tooth and the denture. Supporting teeth crown height of which is reduced by 2-3 mm and which are prepared in a convex or dome form are used. In these supporting teeth, endodontic treatment is mostly necessary, and amalgam or composite is placed in the coronal section of the tooth in the last process. The shortened crown improves the crown-root ratio, thereby decreasing the mobility of the abutment teeth under an overdenture.
