Propriosepsiyon, diz stabilitesine ve fonksiyonuna katkıda bulunan önemli bir mekanizmadır. Ön çapraz bağ
(ÖÇB) yaralanmaları, n.romüsküler sistem içindeki kompleks nöral bağlantıları bozarak propriyosepsiyonun ve
diz eklem stabilitesinin bozulmasına neden olabilir. Bu çalışmanın amacı, ÖÇB rekonstrüksiyonunu takip eden iki
yıl geçtikten sonraki dönemde diz eklem propriyoseptif fonksiyonlarının araştırılmıştır. Bu amaçla, ÖÇB
rekonstrüksiyonu geçirmiş bireylerin yaralanmış ve sağlıklı diz eklemlerinin posizyon hissi karşılaştırılmıştır. Bu
araştırmada, diz eklem propriyosepsiyonu 32 katılımcı üzerinde .l.ülmüştür: 16 ÖÇB rekonstrüksiyonu geçirmiş
birey (yaş (yıl) 26.3±6.5; boy (cm) 174.1±7.3; kilo (kg) 74.1 ± 13.1) ve 16 yaş ve boy uyumlu kontrol grubu (yaş
(Yıl) 24.5 ± 4.2; boy (cm) 173.8 ± 7.3 kilo (kg) 67.7 ± 14.6). Diz eklemi, ÖÇB rekonstrüksiyon grubu için yaralı
ve sağlıklı diz, kontrol grubu için ise sağ ve sol diz olarak sınıflandırıldı. Çalışmaya, ÖÇB rekonstrüksiyonundan
sonra ortalama 3.65 ±0.8 yıl geçmiş kişiler dahil edildi. Propriosepsiyon testleri, Biodex Multi-Joint System 4 Pro
cihazı kullanılarak yapıldı. Testlere başlamadan önce, her katılımcıya, gerekli bilgilendirmeler s.zlü olarak
anlatıldı ve katılımcılara gözleri açık olarak bir örnek deneme yaptırıldı. Katılımcılardan izokinetik cihaza, diz
eklem açısı 90° fleksiyonda iken oturmaları istendi. İlk olarak, katılımcı dizini 60°/s-1 açısal hızda iken aktif
ekstansiyon yaparak hedef açıya (60°, 45° veya 30°) getirmesi istendi ve propriyoseptif öğrenme amacıyla bu
pozisyonda 10 saniye bekletilmiştir. Daha sonra, katılımcılardan diz eklem açısının belirlenen hedef açıda
olduğunu düşündüğünde dinamometreyi durdurmak için elindeki durdurma düğmesine basması istendi. Herhangi
bir öğrenme etkisinden kaçınmak için seçilen 3 hedef açının (60°, 45° ve 30°) sırası her bir diz için randomize
olarak değiştirildi. Her bir açı için ü. deneme yapıldı. Ardışık ü. testin ortalama değeri kaydedildi. Bu ortalama
değerin gerçek açıdan çıkarılmasıyla "açısal hata" hesaplandı. Hatanın pozitif veya negatif olmasına
bakılmaksızın, "mutlak açısal hata" değerlendirildi. Propriosepsiyon test sonuçlarının Shapiro-Wilk normallik
testine göre normal dağıldığı saptanmıştır. Bu nedenle, grup içi karşılaştırılmada eşleştirilmiş örneklem T testi
kullanılmıştır. Ön çapraz bağ, rekonstrüksiyonu geçirmiş taraftaki propriosepsiyon test sonuçları, 60°, 45° ve
30°’lik hedef açılar için sırasıyla 57.5 (± 2.6), 42.8 (±3.6), 27.9 (±3.8) olarak bulunmuştur. Sağlıklı dizde de aynı
hedef açılar için sırasıyla 58,2 (±3,1), 42,5 (±2,6), 28,8 (±3,7) değerleri bulunmuştur. Bu verilere göre, ÖÇB
rekonstrüksiyon grubunda, yaralanmış ve sağlıklı diz eklem propriosepsiyon değerleri arasında istatistiksel olarak
anlamlı düzeyde bir fark bulunmamıştır (p>0,05). Kontrol grubunda, sağ tarafta 60°, 45° ve 30° 'lik hedef açılar
için sırasıyla 60,4 (±3,7), 44,8 (±3,0), 30,6 (±3,1) değerleri bulunmuştur. Sol taraf içinde aynı hedef açılardaki
.l.üm sonuçları sırasıyla 59,3 (±2,9) 44,2 (±2,9), 29,9 (±3,0) olarak bulunmuştur. Kontrol grubunda da sağ ve sol
diz eklem propriosepsiyon değerleri arasında istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05). Bu
çalışmanın sonuçlarına göre, ön çapraz bağ rekonstrüksiyonu geçirmiş bireylerin yaralanmış ve sağlıklı diz eklem
proprisepsiyon değerleri arasında farklılık bulunmamıştır. Aynı şekilde boy ve yaş uyumlu kontrol grubunun da
sağ ve sol diz proprisepsiyon değerleri benzer bulunmuştur. Ön çapraz bağ cerrahisinden sonra rehabilitasyon
sürecine dahil olan bireylerin uzun vadede (ort. 3,65 yıl) aktif eklem posizyon hissi değerlendirildiğinde, sağ ve
sol dizlerinin proprisepsiyon değerlerinin benzer olduğu gözlenmiştir. Diz eklem propriosepsiyonun yaralanma
riskini etkileyen bir faktör olduğu göz .nüne alındığında, ameliyat sonrası uygulanan rehabilitasyon
programlarının uzun vadede yaralanma riskini azaltabileceği g.rülmektedir.
Proprioception is a major mechanism contributing to knee stability and function. Anterior curiciate ligament
(ACL) injuries can disrupt complex interactions within the neuromuscular system, resulting in impaired
proprioception and knee joint stability. The aim of our study was to investigate knee proprioceptive functions after
2 years from ACL reconstruction. Thus, we compared the knee joint position sense among subjects in the healthy
and ACL injured knee after the reconstruction of ACL. Proprioception of the knee was measured in 32 participants;
16 with reconstructed ACL (age (year) 26.3±6.6; height (cm) 174.1±7.3 weight (kg) 74.1±13.1) and 16 age and
height matched controls; age (year) 24.5±4.2; height (cm) 173.9±7.26 weight (kg) 67.7±14.6). Knee joints
classified and evaluated as injured and healthy knee for ACL reconstructed group and as right and left knee for the performed at Biodex Multi-Joint System 4 Pro. The instructions were given to each participant before the test and
familiarization trials were performed with their eyes open. The participants were asked to sit on the device, his/her
knee relaxed at 90° of flexion at the beginning. First, the participant extended the knee actively to the targeted
angle (60°, 45° or 30°) at 600/s and paused for 10 seconds for proprioceptive learning. Then, the patient was
instructed to reproduce the targeted knee extension angles of 60°, 45° and 30° separately. The participants pushed
the button to stop the dynamometer when he/she thought his/her knee angle was at the designated target angle.
The order of the 3 selected target angles was randomized to avoid any learning effect. The participants were asked
to perform three trials for each angle. The mean value of the three successive tests was recorded. The "angular
error" was calculated by subtracting this average value from the real angle. Regardless of whether the error is
positive or negative, "absolute angular error" is evaluated. It was observed that the obtained data were normally
distributed according to Shapiro-Wilk normality test. Therefore, paired sample T-test was applied for within group
comparison. The angular results of the proprioception test were found to be 57.6 (±2.6), 42.8 (±3.6), 27.9 (±3.8)
on the reconstructed side of the experimental group for the targeted angles of 60°, 45° and 30° respectively. On
the healthy side the results were as follows; 58.2 (±3.1), 42.5 (±2.6), 28.8 (±3.7) for the targeted angles. According
to these findings, no statistically significant difference was found between the reconstructed and the healthy side
in the experimental group (p>0.05). In the control group the angular results of the proprioception test were found
to be 60.4 (±3.7), 44.8 (±3.03), 30.6 (±3.05) on the the right side for the targeted angles of 60°, 45° and 30°
respectively. Angular values are found on the left side 59.3 (±2.9), 44.2 (±2.9), 29.9 (±3.0) for the targeted angles.
No statistically significant difference was found between the right and left sides of the control group (p>0.05). In
this study, there was no significant difference between injured and healthy knee in ACL reconstructed individuals
and height and age-matched controls. It was observed that the active joint position sense of those who underwent
rehabilitation was similar between the reconstructed and healthy knees after a long period from ACL operations
(mean 3.65 years). As a result, it seems that the rehabilitation program applied after surgery can reduce the risk of
injury in the long term, considering that knee proprioceptive sense is a factor affecting the risk of injury.
