Çoklu Diş Eti Çekilmesinin Modifiye Koronale Kaydırılan Flep ve Bağ
Doku Grefti ile Tedavisi: Bir Olgu Sunumu

Özet
GiriĢ: DiĢ eti çekilmeleri (DÇ), diĢ etinin mine-sement sınırının apikalinde konumlanması ile oluĢan
tekli veya çoklu Ģekilde görülebilen mukogingival durumlardır. Cerrahi sahanın büyüklüğü dolayısıyla
alınan greftin büyük olması, flep stabilizasyonunun daha zor olması, post operatif ödem ve hemoraji
riskinin artması gibi nedenlerle çoklu DÇ (ÇDÇ) tekli çekilmelere göre baĢarılı tedavisi daha zordur.
Günümüzde koronale kaydırılan flep (KKF) ile bağ doku greftinin (BDG) kombine kullanımı kök
yüzeyi kapaması için altın standart tedavi olarak kabul edilmiĢtir. Bu vaka raporunda ÇDÇ‟nin
modifiye KKF ve BDG ile tedavisi sunulmaktadır. Vaka: Sistemik sağlıklı 29 yaĢındaki erkek hasta
üst çene 24-25-26 numaralı diĢlerde sırasıyla 2 mm, 1mm ve 3 mm boyutunda Miller sınıf I ÇDÇ
kaynaklı hassasiyet Ģikayeti ile kliniğimize baĢvurdu. Hastaya ağız hijyeni eğitimi, diĢ taĢı temizliği ve
kök yüzeyi düzleĢtirmesini kapsayan baĢlangıç periodontal tedavi uygulandı. 4 hafta sonra lokal
anestezi altında modifiye KKF ve BDG tekniği uygulandı. BDG‟nin sabitlenmesi ve ardından flebin
suture edilmesi için 5.0 rezorbe olabilen dikiĢ kullanıldı. Post operatif herhangi bir komplikasyona
rastlanmadı. Operasyondan 2 hafta sonra dikiĢler alındı. Sonuç: Bu olgu sunumunda, maksilladaki
ÇDÇ‟nin modifiye KKF ve BDG tekniği uygulaması ile 3 ay takipli estetik ve fonksiyonel olarak
baĢarılı tedavisi gösterilmiĢtir.

Treatment of Multiple Gingival Recession Defects With Modified Coronally Advanced Flap
Technique and Connective Tissue Graft: A Case Report
Abstract
Introduction: Gingival recessions (GR) are conditions that occur with the gingiva positioned at the
apical of the cemento-enamel junction. It can be seen single or multiple. Multiple gingival recessions
(MGR) are more difficult to treat due to the large graft harvested, the more difficult flap stabilization,
and the increased risk of post-operative edema and hemorrhage, compared to single recessions. Today,
coronally advanced flap (CAF) technique used in combination with connective tissue graft (CTG) is
the gold standard for root coverage. In this case report, treatment of MGR with modified CAF
technique and CTG is presented. Case: A systemically healthy 29 years old male patient was admitted to our clinic with the complaint of sensitivity due to Miller class I MGR in the upper jaw teeth 24-25-
26. The recessions depths were 2 mm, 1 mm and 3 mm, respectively. Non surgical periodontal
treatment including oral hygiene instruction, scaling and root plaining was applied to the patient. 4
weeks later, modified CAF technique and CTG were performed under local anesthesia. 5.0 resorbable
sutures were used to fix the CTG and then suture the flap. No postoperative complications were
observed. Sutures were removed 2 weeks after the operation. Conclusion: In this case report an
aesthetically and functionally successful treatment of MGR with the application of modified CAF
technique and CTG in the maxilla with 3 months follow-up has been demonstrated.
