Oral antiseptik ajanların Candida Albicans’ın fosfolipaz ve proteinaz enzimleri üzerine etkisi

Özet:

1. ORAL ANTİSEPTİK AJANLARIN CANDIDA ALBICANS’IN FOSFOLİPAZ VE PROTEİNAZ ENZİMLERİ ÜZERİNE ETKİSİ Banu Uygun-Can Candida albicans, ağız mukozasına kolonize olabilen fırsatçı bir patojendir. Bazı lokal ve sistemik faktörlerin etkisi ile sayısı artarak, çeşitli oral kandidiyaz tiplerine sebep olmaktadır. Bunların başında Candida ilişkili protez stomatiti gelmektedir. Genel olarak bu tür enfeksiyonların tedavisinde antiseptik içeren ağız gargaraları, antifungal tedaviye alternatif olarak kabul edilmektedir. Yapılan çalışmalar, ağız içindeki antiseptik konsantrasyonunun ağız dinamiklerinin etkisiyle azaldığını göstermektedir. Bu durum, tedavi sırasında düşük antiseptik konsantrasyonunun Candida’nın karakterini veya patojenitesini nasıl etkilediği sorusunu akla getirmektedir. Bu nedenle çalışmamızda; protez stomatiti hastalarından izole edilen 20 C. albicans suşunun virulans faktörleri üzerine (fosfolipaz ve proteinaz), tedavide kullanılan klorheksidin glukonat (%0.2), heksetidin (%0.1) ve triklosan (%0.045) olmak üzere 3 antiseptik solusyonunun 4 farklı sub-terapötik konsantrasyonlarının (1/150, 1/200, 1/250, 1/300) etkisinin araştırılması amaçlanmıştır. Fosfolipaz deneyi yumurta sarısı içeren Sabouraud dextroz agar besiyerinde, proteinaz deneyi ise sığır serum albumin içeren besiyerinde yapılmıştır. Üç antiseptik ajanın 4 farklı sub-terapötik konsantrasyonu ile 30 dakika temas edilen 20 suşun fosfolipaz ve proteinaz aktivite değerlerinin, aynı suşların antiseptik ile temas edilmeyen (kontrol) değerlerine göre anlamlı (p < 0.05) olarak azaldığı bulunmuştur. Üç antiseptiğin kendi sub-terapötik konsantrasyonları arasındaki değişim anlamlı (p < 0.05) bulunmuştur. Bu antiseptiklerin aynı konsantrasyonları karşılaştırıldığında ise enzim düzeyleri arasında, anlamlı bir fark bulunmamıştır (p  0.05). Literatür ve bulgularımızın değerlendirilmesi sonucunda; Candida ilişkili protez stomatiti tedavisinde kullanılan oral antiseptik ajanların sub-terapötik konsantrasyon değerlerinin, C. albicans’ın virulans faktörlerinin aktivitelerini azaltarak, suşun patojenik etkisini azaltabileceği veya inhibe edebileceği düşüncesindeyiz. SUMMARY Effect of Oral Antiseptic Agents on Phospholipase and Proteinase Enzyms of Candida albicans Banu Uygun-Can Candida albicans, an opportunistic pathogen that can colonize the oral mucosa. Some local and systemic factors increases the number of C. albicans which causes various types of oral candidiasis. The most common oral candidiasis type is Candida-associated denture stomatitis. In generally, oral rinse with antiseptics which were used as treatment of these types of infections, was considered as alternative of antifungal treatment. Studies have suggested that the intraoral concentrations of antiseptic decrease considerably due to the dynamics of the oral cavity. This condition yield the question of how low antiseptic concentrations affect the character or pathogenesis of Candida during treatment. Therefore the aim of this study was to investigate the effect to four sub-therapeutic concentrations (1/150, 1/200, 1/250, 1/300) of mainly chlorhexidine gluconate (0.2%), hexetidine (0.1%) and triclosan (0.045%) on the virulance factors (phospholipase and proteinase) of C. albicans isolates from 20 patients with denture stomatitis. Phospholipase test was done on Sabouraud dextrose agar with egg yolk, proteinase test was done on the bovine serum albumin agar. Phospholipase and proteinase activity values of 20 strain which were brief exposured (30 min) to four different sub-therapeutic concentrations of three antiseptic agents decreased significantly (p<0.05) when it was compared with the strains that were not exposured with antiseptics (control) values. There is significant (p<0.05) difference between the self sub-therapeutic concentrations of the three antiseptics. When the same concentrations of each antiseptic was compared, there is no significant (p>0.05) difference between enzymatic activities. As a result of literature research and the studies, it was thought that sub-therapeutic concentration levels of oral antiseptic agents which are used on treatment of Candida-associated denture stomatitis may reduce or inhibit pathogenicity effect of strain by reducing the virulance factors of C. albicans.
