Hematopoetik kök hücre nakli planlanan olgularda kardiyopulmoner ve biyokimyasal parametreler arasındaki ilişkinin incelenmesi

Özet:

Amaç: Hematopoetik kök hücre nakli alıcılarında (HKHN) solunum fonksiyon testi ve submaksimal aerobik kapasite pretransplant tetkikleri arasına girmiştir. Bununla birlikte, literatürde açık olmayan akut faz reaktanları (C-Reaktif protein, ferritin, fibrinojen, albümin, ESR) ve hemogram (hemoglobin, platelet) değerlerinin kardiyopulmoner parametrelere etkisinin incelenmesi planlanmıştır. Gereç ve Yöntem: 94 HKHN alıcısının verileri retrospektif olarak incelenmiştir. Akut faz reaktan seviyeleri ve hemogram değerleri, hasta dosyaları ve bilgi yönetim sisteminden alınıp pulmoner fonksiyonları (spirometre), egzersiz kapasiteleriyle karşılaştırılmıştır. Bulgular ve sonuçlar: Allojeneik hematopoetik kök hücre nakli (AHKHN) alıcılarında pulmoner fonksiyon ile CRP (p<0,01) anlamlı ilişkilidir. Ferritin ve fibrinojen seviyeleri (p<0,01) ile aerobik kapasiteyi öngörmede kullanılabilir. Otolog hematopoetik kök hücre nakli (OHKHN) alıcılarında CRP seviyesi (p<0,05) aerobik kapasiteyle ilişkilidir. Kadın alıcılarda FEV1 ve FVC parametrelerini öngörmede (p<0,05) CRP değeri kullanılabilir ve genel kas kitlesi CRP ve ESR ile (p<0,01) ve fibrinojen ile (p<0,05) öngörülebilir. Lenfoma tanılı hastalarda FEV1 ve FVC, ESR ile (p<0,05) ve aerobik kapasite ESR ve ferritin ile(p<0,05) ilişlidir. Myelom tanılı hastalarda FEV1 ve FVC fibrinojen ile (p<0,01) ilişkiyken aerobik kapasite Hb (p<0,001), ferritin (p<0,01) ve CRP ile (p<0,01) ilişkilidir. Akut lösemi tanılı hastalarda CRP FEV1(p<0,01) ve FVC (p<0,05) ile anlamlı ilişkideyken aerobik kapasite ferritin(p<0,01), fibrinojen (p<0,001) ve CRP (p<0,01) ile öngörülebilir. Objectives: In hematopoietic stem cell transplant(HSCT) recipients, pulmonary function test and submaximal aerobic capacity measurement were among the pretransplant tests. However, the aim of this study was to investigate the impact of hemogram (hemoglobin,platelet) and acute phase reactant (CRP, ferritin, fibrinogen, albumin, ESR) values on cardiopulmonary parameters in hematopoietic stem cell transplant recipients. Metarials and Methods: The data of 94 HSCT recipients were analyzed retrospectively. Acute phase reactant levels and hemogram values were taken from patient files and information management system and compared with pulmonary functions (spirometry) and exercise capacity (6mwt). Findings and Results: Pulmonary function and CRP (p<0.01) associated with allogeneic hematopoietic stem cell transplantation (AHSCT) recipients. Ferritin and fibrinogen levels (p<0.01) can be used to predict aerobic capacity. CRP levels (p<0.05) were associated with aerobic capacity in autologous hematopoietic stem cell transplant (OHSCT) recipients. In female, CRP values can be used to predict FEV1 and FVC parameters (p<0.05) and overall muscle mass can be predicted by CRP and ESR (p<0.01) and fibrinogen (p<0.05). FEV1 and FVC were associated with ESR (p<0.05) in patients with lymphoma and aerobic capacity with ESR and ferritin (p<0.05). FEV1 and FVC were associated with fibrinogen (p<0.01) in patients with myeloma , whereas aerobic capacity was associated with Hb (p<0.001), ferritin (p<0.01) and CRP (p<0.01). CRP was significantly associated FEV1 (p<0.01) and FVC (p<0.05) in patients with acute leukemia, whereas aerobic capacity ferritin (p<0.01), fibrinogen (p<0.001) and CRP (p<0.01).
